Important Changes to Community Grant Application Process 

Announcing the 2024-2026 Health Impact Grantees


We believe that we can be more effective in achieving outcomes on the most critical community health issues by aligning our strategies and investments as funders. The current siloed funding process and reporting requirements of multiple funders strain the capacity of many non-profit providers away from delivering the services needed to address critical health and social needs. By combining our community health funding resources, we aspire to work together with community partners more effectively while decreasing the administrative burden, strengthen the system of care in our community, and achieve greater impact in health outcomes for the residents of Santa Fe and northern New Mexico.
Across northern New Mexico, hundreds of nonprofits are working day and night to heal and provide essential care and services. Even when the days are short and resources are tight, they show up for their communities. And we show up for them. The Community Health Funder Alliance represents leading funders in northern New Mexico focused on improving health outcomes and promoting health equity. As an Alliance, we support nonprofits as they do vital work on the ground. From grantmaking, to sharing expertise and connections, to learning together as partners in the field, we work hand-in-hand, taking action together to promote care and advance the health of our communities.

The Community Health Funder Alliance is dedicated to improving health and wellness in Santa Fe and northern New Mexico by strengthening the systems of care that address the most challenging health and social needs in our community.

Through aligned goals, strategies, and grants, the Alliance helps further solutions that heal, care for, and sustain healthy people and communities.

By supporting work that addresses the community’s immediate health needs as well as the larger system of care, the Alliance is a coordinated effort to impact the health outcomes of community members in Santa Fe and northern New Mexico.


Who are we?


The Community Health Funder Alliance is a collaborative focused on improving systems of care and promoting health equity in Santa Fe and northern New Mexico. The Alliance is made up of Anchorum Health Foundation and CHRISTUS St. Vincent (CSV). We support Alliance Health Impact Grantees throughout northern New Mexico.

Our mission is to design and implement a unified funding strategy to enhance and strengthen systems of care that address the most challenging health and social needs in Santa Fe and northern New Mexico.  We achieve our mission in partnership with the community.

Community Partners


Community partners include the Santa Fe Community Foundation, City of Santa Fe, and Santa Fe County as well as the Alliance Health Impact Grantees throughout northern New Mexico. Together we are advancing the health of our communities.

Our Funding Priorities



The Alliance partners have combined funds to create a pooled tier of funding for Health Impact Grants, focused on health and wellness in the following areas across the lifespan:

  • Maternal Health and Early Childhood
  • School-Age Children and Adolescent Health, Wellness
  • Adult Physical Health
  • Adult Behavioral Health
  • Women’s Health
  • Older Adult Health
  • Social Determinants of Health Across the Lifespan

The geographic region for Health Impact Grants includes Santa Fe and northern New Mexico.

Health Impact Grants are invitation-only, multi-year grants awarded to programs that significantly contribute to turning the curve on impact goals and are able to scale services to address critical needs. 

This dynamic collaboration between funders and nonprofits has helped reduce barriers in the systems of care, increased sustainability for service providers, and improved outcomes for individuals and families.

One of our key goals is to strengthen the system of care that supports our most vulnerable residents. It has been a long standing dream to have funders coordinate their funding processes to ease administrative burdens on service providers so they can focus on the important business of delivering care.
