The Community Health Funder Alliance is dedicated to improving health and wellness in Santa Fe and northern New Mexico by strengthening the systems of care that address the most challenging health and social needs in our community.

The Alliance targets health outcomes in the following areas across the life span using CHRISTUS St. Vincent’s Community Health Needs Assessment (CHNA) and the Results-Based Accountability (RBA) approach.

Funding Strategies and Criteria


Health Impact Grants are invitation-only, multi-year grants awarded to programs that significantly contribute to turning the curve on impact goals and are able to scale services to address critical needs or serve as sole provider of services. Unfortunately, new applications for Health Impact grants are not being considered at this time.

In July 2023, the second cohort of Health Impact grantees were awarded multi-year grants for three 12-month terms.

Nearly $3 million was awarded for FY 23-24.

Key funding criteria for Health Impact Grants include:

    • Existing programs or organizations that meet the following criteria:
      • Demonstrated capacity to influence outcomes (contribute to turning the curve on impact goals) and scale services to address critical needs identified in the Community Health Needs Assessment
      • History of demonstrated performance outcomes over a three year period
      • Critical safety net or sole provider of services
    • Demonstration of collaborative leadership to address critical needs
    • Programs serving populations in Santa Fe and northern New Mexico.
