In July 2023, the Community Health Funder Alliance announced its 2024-2026 Health Impact Grantees. The Alliance awarded over $2.8M for fiscal year 2023-2024 to 17 community organizations working to improve health and wellness in Santa Fe and northern New Mexico.

Maternal Health and Early Childhood
2023-2024 Grant Awards Total $380,000

Grants are supporting the following:

  • Programs that address risk factors that affect healthy births and healthy early development.
  • OB and Medication-Assisted Treatment services for pregnant women and women addicted to opioids.
  • Community navigation that connects families to wraparound care and community resources
  • Integrated community support for children and families facing social, emotional or developmental challenges and trauma.
  • Intergenerational family coaching to support families with young children.
Children and Adolescent Health
2023-2024 Grant Awards Total $470,000

Grants will provide important funding to support the following:

  • Wrap-around support, case management and counseling services for children and adolescents experiencing homelessness, adverse experiences or trauma.
  • Mental health services for young children and their families.
  • Youth suicide prevention services and intensive support for adolescents at-risk for suicide.
  • Health care including dental and behavioral health services for children and teens.
Adult Behavioral Health
2023-2024 Grant Awards Total $957,500

Grants are supporting the following:

  • Counseling, peer support and recovery services for individuals seeking sobriety.
  • Harm reduction services to prevent overdose deaths and improve health outcomes for individuals who use substances.
  • Affordable, accessible client-centered behavioral healthcare services and support.
  • Emergency crisis response.
Women’s Health
2023-2024 Grant Awards $205,000

Grants will provide important funding to support the following:

  • Trauma-informed domestic violence, sexual violence and child abuse and neglect programs including 24/7 hotlines, shelter services, transitional housing, education, legal and medical advocacy, counseling & support groups, and children’s programs.
  • Violence prevention and support for victims of sexual assault.
Older Adult Health
2023-2024 Grant Awards Total $90,000

Grants are supporting the following:

  • Healthy, nutritious meals for chronically homebound individuals.

Grantees: 

Social Determinants of Health - Housing and Homelessness
2023-2024 Grant Awards Total $700,000

Grants are supporting efforts including the following:

  • Street outreach and emergency shelter services for youth and adults.
  • Medical respite for patients discharged from the hospital with no home to return to.
  • Transitional living programs, case management, education and support services for homeless youth and adults.
  • Permanent supportive housing for individuals who are homeless.
  • Meal service, food distribution, and referral and navigation assistance with housing, transportation and access to medical care.
  • Eviction prevention and advocacy.

Pregnant women and their families need services now more than ever. The CHFA Health Impact Grant helps Las Cumbres to stabilize our current programs, and expand capacity. The decrease in administrative burden from combined application and reporting with meaningful accountability that supports our programs goals means that we can continue to focus on the essential work of providing care to families who need us.
