										<content:encoded><![CDATA[<p><strong>Santa Fe, New Mexico</strong> — The Community Health Funder Alliance has awarded more than $3 million in grants this year, including three new mid-cycle grantees, to support the health and well-being of northern New Mexico residents. This continued investment brings the total funds awarded by the Alliance to over $8.6 million since its inception in 2019.</p>
<p>This year&#8217;s funding expansion reﬂects the Alliance&#8217;s commitment to ensuring older adults can safely age in place in their homes. Three new nonproﬁt partners—Coming Home Connection, Life Circle Adult Day Center, and Villages of Santa Fe—have been added to the roster of grantees, all of which are focused on providing critical support and services to improve community health across northern New Mexico.</p>
<p>“Our mission is to design and implement a uniﬁed funding strategy to enhance and strengthen systems of care that address the most challenging health and social needs in Santa Fe and northern New Mexico,” said Lillian Montoya, President and Chief Executive Oﬃcer of CHRISTUS St. Vincent. “By collaborating with community foundations and other partners, we’re able to amplify our impact and better serve our communities.”</p>
<p>The Alliance is a collaboration between two key funders—Anchorum Health Foundation (AHF) and CHRISTUS St. Vincent (CSV)—working together to build a more equitable and supportive system of care in northern New Mexico.</p>
<p>“We’ve made signiﬁcant progress since the Alliance’s launch in 2019,” said Jerry Jones, President and CEO of Anchorum Health Foundation. “Through our joint eﬀorts, we’ve built a stronger system of care, and the inclusion of new grantees demonstrates our ongoing dedication to improving health outcomes across the region.”</p>
<p>With the 2024 expansion, the focus on serving the older adult community in northern New Mexico has been signiﬁcantly strengthened by the addition of three exceptional organizations: Coming Home Connection, Life Circle Adult Day Center, and Villages of Santa Fe. These organizations are dedicated to enhancing the lives of older adults in the region. Coming Home Connection provides compassionate in-home caregiving, equipment loans and resources to older adults and their families. Life Circle focuses on improving the well-being of elders through socialization and respite</p>
<p>care, especially for those living with dementia. Villages of Santa Fe promotes a healthy, active lifestyle for seniors through a variety of exercise and balance classes, available both in-person and online, aimed at improving physical and mental well-being.</p>
<p>“Coming Home Connection is very grateful for this grant from the Community Health Funder Alliance,” said Ellen London, CEO of Coming Home Connection. “Their generous support will allow us to continue to serve older adults in our community and to extend our services to those in need of care in the city and beyond.”</p>
<p>The Community Health Funder Alliance goes beyond grantmaking by sharing expertise, building strategic partnerships, and streamlining processes to support nonproﬁts in their essential work. Over the past three years, the Alliance has awarded over $8.6 million to more than 40 community health organizations, ensuring they have the resources needed to care for and heal their communities, even in challenging times.</p>

This is the second cycle of multi-year funding, increasing available funds by over 50 percent from last year.</p>
<p>Since its launch in 2019, the Community Health Funder Alliance has invested more than $8.4 million to enhance the health for northern New Mexico residents. Through grantmaking and vital partnerships, the Alliance helps further solutions that heal, care for, and sustain healthy people and communities. Alliance members include Anchorum St. Vincent (ASV) and CHRISTUS St. Vincent (CSV).</p>
<p>“Our grantees are addressing the most challenging health and social needs in our community,” said Lillian Montoya, President and Chief Executive Officer of CHRISTUS St. Vincent. “Through their work, they are immediately serving community members in need, and also improving the larger system of care to make it better for our residents in the long run.”</p>
<p>Together, the Alliance supports nonprofits in health and wellness across the lifespan–from maternal health and early childhood to school-age children, adolescent, adult, and older adult health services. The Alliance also invests in social determinants of health through housing and the unhoused.</p>
<p>The Alliance’s grant making initiatives are multi-year Health Impact Grants that build momentum for long-term, far-reaching solutions.</p>

<p><span style="font-weight: 400;">Over the last three years, Community Health Funder Alliance community partners and organizations have worked day and night to heal and provide essential care and services. The COVID pandemic significantly intensified needs across our communities, and community organizations stepped up. With skills and with heart, organizations have provided the critical safety net and services needed in communities across northern New Mexico. At the Alliance, we have been honored and grateful to be a part of supporting this important work. </span></p>
<p><span style="font-weight: 400;">As we enter our fourth year as a funding collaborative, we continue to dedicate ourselves to learning and responding to our community’s needs. In 2023 the Alliance and its partners are adapting to further solutions that heal, care for, and sustain healthy people and communities. There are important changes on the horizon to respond to 2023 CHRISTUS St. Vincent Community Health Needs Assessment and to evolve and strengthen our partnerships.</span></p>
<p><span style="font-weight: 400;">Beginning in 2023, Community Grants will be managed by the Santa Fe Community Foundation (SFCF). Grantees can apply through SFCF.  Positioning Community Grants within SFCF will allow for additional fundraising opportunities. While Community Grant funds will be managed by Santa Fe Community Foundation, Anchorum and CHRISTUS St. Vincent will continue as active partners with SFCF. </span></p>
<p><span style="font-weight: 400;">The Alliance will continue to support health impact strategies, guided by the new 2023  Community Health Needs Assessment. This includes a goal to strengthen the system of care through continued health impact funding and community partnerships. </span></p>

										<content:encoded><![CDATA[<p><b>Santa Fe, New Mexico</b><span style="font-weight: 400;"> – The Community Health Funder Alliance furthers its commitment toward greater health equity and improving health outcomes in northern New Mexico by awarding 59 nonprofit organizations with $1.9 million in grants. This current round of grants brings the total investment in health outcomes in northern New Mexico to $5.5 million over the last three years.</span></p>
<p><span style="font-weight: 400;">&#8220;The nonprofits we are supporting are making a significant impact in people’s lives by providing greater access to health services,&#8221; said Alliance member Christopher Goett, President and CEO, Santa Fe Community Foundation. &#8220;We continue to hear very moving first-hand accounts from New Mexicans about how The Alliance’s grantmaking is advancing the work of nonprofits who, since the beginning of the pandemic, have expanded their services to meet the urgent needs of our community,” said Lillian Montoya, President and CEO, CHRISTUS St. Vincent. </span></p>
<p><span style="font-weight: 400;">The Alliance&#8217;s grantmaking includes annual Community Grants ($5,000 to $15,000) to sustain efforts and multi-year Health Impact Grants ($20,000 to $175,000) to build momentum for long-term, far-reaching solutions. </span></p>
<p><span style="font-weight: 400;">&#8220;Having funders like The Alliance allows us to continue our work of serving essential meals to our community members,&#8221; said Tony McCarty, Executive Director, Kitchen Angels. &#8220;We delivered a meal to a person dealing with a difficult situation recently and he shared that the meal ‘was so fantastic, you could just tell that whoever put this together really cared about what they were doing.’  These grants allow us to continue to serve people just like this individual.&#8221;</span></p>

This year’s cycle is open from April 1</span><span style="font-weight: 400;">st</span><span style="font-weight: 400;"> to April 30</span><span style="font-weight: 400;">th</span><span style="font-weight: 400;">. Community Grants are annual, open, competitive grants between $5,000 and $15,000. These grants, totaling $425,000 this year, will be awarded to new and existing community health programs that enhance and strengthen the system of care in northern New Mexico. </span></p>
<p><span style="font-weight: 400;">An additional </span><span style="font-weight: 400;">$1,390,000</span> <span style="font-weight: 400;">will be awarded to Health Impact Grantees for the third year of their three- year grant awards. </span><span style="font-weight: 400;">To date, the Alliance has awarded grants to 54 nonprofits through its Community Health and Health Impact programs – a current total investment in health outcomes in northern New Mexico at $3.6 million.</span></p>
<p><span style="font-weight: 400;">Since 2019, The Community Health Funder Alliance has been instrumental in supporting organizations and New Mexico communities. The Alliance helps further solutions and outcomes through grantmaking and partnerships while promoting health equity. Members currently include Anchorum St. Vincent, CHRISTUS St. Vincent, and the Santa Fe Community Foundation. </span></p>
<p><span style="font-weight: 400;">“One of our key goals is to strengthen the system of care that supports our most vulnerable residents, especially as our northern New Mexico communities recover from the effects of the pandemic,” said Lillian Montoya, President &amp; CEO of CHRISTUS St. Vincent. </span></p>

<p><span style="font-weight: 400;">“</span><span style="font-weight: 400;">Across northern New Mexico, hundreds of nonprofits are working day and night to heal and provide essential care and services in communities,” said Lillian Montoya, President and Chief Executive Officer of CHRISTUS St. Vincent. </span><span style="font-weight: 400;">“</span><span style="font-weight: 400;">These leaders show up for their communities. <p><span style="font-weight: 400;">Together, the Alliance supports nonprofits in health and wellness across the lifespan: from maternal health and early childhood to school-age children, adolescent, adult and senior health services. The Alliance also invests in social </span><span style="font-weight: 400;">determinants of health including housing and homelessness, healthy food, and healthy neighborhoods.</span></p>
<p><span style="font-weight: 400;">The Alliance</span><span style="font-weight: 400;">’</span><span style="font-weight: 400;">s grant making includes annual Community Grants ($5,000 to $15,000) to sustain efforts, and multi-year Health Impact Grants ($20,000 to $175,000 per year) to build momentum for long-term, far-reaching solutions. </span></p>

The Community Health Funder Alliance released $1.8 million in grants in 2020, its ﬁrst year of annual collaborative grantmaking. These resources arrive at a critical time for health organizations in the northern region of our state as we continue to grapple with the challenges of COVID-19.</p>
<p>Las Cumbres Community Services, an integrated, trauma-informed Child and Family Services organization operating throughout northern New Mexico, is one of this year’s 44 grantees. “Pregnant women and their families are especially vulnerable during the pandemic and need services now more than ever,” said Stacey Frymier Interim Executive Director of Las Cumbres. “The Community Health Funder Alliance grant frees resources to focus on the essential work of providing care to families.”</p>
<p>The Community Health Funder Alliance includes Anchorum St. Vincent, CHRISTUS St. Vincent, and the Santa Fe Community Foundation. The Alliance was founded prior to the pandemic, but its relevance to supporting organizations and communities through challenging times is underscored by the impacts of COVID-19.</p>
<p>The $1.8 million dollars in grant funds for the 2020-2021 grant cycle were distributed through annual Community Grants of $5,000 to $15,000 for general operating support, and through larger scale Health Impact Grants ranging from $20,000 to $175,000. Larger scale grants support multi-year programs and strategic partnerships.</p>
<p>“It is essential that nonproﬁt providers have the capacity to deliver the services needed to address critical health and social needs, especially now,” Said Lillian Montoya, President and Chief Executive Oﬃcer of CHRISTUS St. Vincent. “As an Alliance we are working together to strengthen the system of care and achieve greater impact on health outcomes across northern New Mexico from multiple angles.”</p>
<p>Together, the Alliance supports nonproﬁts in health and wellness across the lifespan: from maternal health and early childhood to school-age children, adolescent, adult and senior health services. <p>“Our health and social support systems are complex and interconnected,” said Peter Bastone, President and CEO of Anchorum St. Vincent. “Alongside providing funds, our Alliance has streamlined the application process to save nonproﬁts valuable time, and we will promote stronger partnerships and better results for the programs that are improving health outcomes in our communities.”</p>
<p>Now, instead of completing three separate grant applications, contracts, and reports, organizations complete just one. “For us, this means our staﬀ and volunteers have more time each year to serve grieving families and for that we are deeply grateful!&#8221; said Katrina Koehler and Nicole Maes-Gonzales, co-executive directors of Gerard&#8217;s House, a center for grieving children in Santa Fe among the 44 grantees this year.</p>

<p>The Alliance’s mission is to strengthen the system of care in our community and achieve greater impact on health outcomes. Anchorum St. Vincent, CHRISTUS St. Vincent, and the Santa Fe Community Foundation coming together doesn’t only mean $1.8 million resources for nonproﬁts and programs. It means that those who are working in community have less of an administrative burden in applying for funds. Additionally, the Alliance aims to build connections and promote more effective reporting and data collection to meaningfully inﬂuence health outcomes across northern New Mexico.</p>
